New Patients
Our comprehensive consultation is designed to gather clinical and biochemical information in order to provide individualized treatment recommendations.
The consultation is completed in two appointments. The first appointment consists of a detailed clinical evaluation with a master’s level clinician. This appointment will review prior medical history, family medical history, and current symptoms. We encourage patients who have background medical testing or relevant cognitive testing to bring copies of these documents with you to the first appointment. The more information available, the more comprehensive and helpful your consultation will be. Please do not bring the original medical records.
Dr. Greenblatt will review that information and will then meet with you at a second appointment to complete a detailed psychiatric evaluation. During the psychiatric evaluation, Dr. Greenblatt will be able to discuss what treatment options might work best for you. This program could include medications, nutritional supplements, or a combination of treatments. Further nutritional testing or a referenced-EEG may also be recommended. First Appointments
Patients are required to complete four forms prior to the first appointment. These forms are
- Patient information sheet
- Patient cover page
- Patient fee schedule agreement.
- Medication lists
Please complete these forms and bring them to your first appointment. Please make copies of all your records prior to your appointment.
Travel Time
Allow extra time for your travel to the first appointment. Dr. Greenblatt has limited appointment time and late arrivals may need to schedule for another day. Detailed directions are included in the link below
Payment
Payment for services is due at the time of the appointment. We accept check, Visa, or MasterCard. We do not accept insurance, though we do provide all patients with receipts which can be submitted for reimbursement of expenses. How out-of-network expenses are handled varies depending on the insurance provider.
Insurance Policy
We are unable to accept insurance at Comprehensive Psychiatric Resources. This policy is largely driven out of practicality as insurance carries with it large burdens and overhead costs. These burdens would lessen the quality of care that we would be able to provide as they would distract staff and physicians from the significant review and attention to detail that is required to provide individualized care.
All patients are provided with receipts that include the necessary information to submit claims to insurance for out-of-network benefits. How out-of-network costs are handled varies depending on the insurance provider.
We are aware of the financial strain this policy can place on families, but we believe that providing individualized care is an essential part of our mission. We strive to make our services affordable and practical with the ambitious goal of minimizing the length of treatment through Integrative Care and Personalized Medicine. After 18 years, we have seen many patients get better and cease to need our services on a regular basis.
